CHART. 2 COLLABORATION ACTIVITIES WITH COMMUNITY PARTNERS
Chart 2 shows collaboration with community partners that our agency has engaged and identifies the proportions of clients refereed to services. Fundejoven has created links with key departments and divisions inside the Arlington Department of Human Services (DHS), developed a survey questionnaire to assess family needs, and provided individual case management services in collaboration with the Arlington DHS. This collaboration has consisted of phone contacts, setting meetings, assisting clients to communicate (translation in some cases), persuade and coach clients on how to navigate the system, and advocate on behalf of client to expedite services. As a result of these case management services and above links, families were able to receive information on or get services such as food stamps, crisis assistance, shelter, housing grants, and medical/dental services from the Career Center and Arlington Mill.
